Material Transfer Agreements

The Adobe Portable Document Format (PDF) documents must be signed by an authorized Technology Transfer representative from your institution. Your browser must be equipped with a PDF reader plug-in or helper program. (Get free download from Adobe.) You may either print a blank form or complete the form electronically, then print. Once printed, the documents must be completed, signed and faxed or mailed to the MMRRC. Instructions specific to each Agreement are in the individual documents. The documents will be counter-signed by a designated representative from the MMRRC and your submission or request will be processed.

Alterations to these Agreements cannot be accepted.
